Frequently Asked Questions
How do I get forms, such as disability, FMLA, etc., completed by my doctor?
You must either mail or bring into the office the form and a HIPAA-compliant patient authorization, which permits your physician to release the information to the requestor. Your physicians then will carefully fill out the form as necessary. We will be happy to mail you a HIPAA-compliant authorization for you to sign in front of a notary if you need one. Most forms require a fee payable at the time the form is received. The expected turnaround time is 7 to 10 business days.
How do I know if START accepts my insurance?
Here at START, we make it convenient for you by accepting most major
insurance plans. Please contact us at our START Center Medical Center location to see if your

Do I need a referral from my doctor to schedule an appointment with START?
It depends on your insurance. Some insurance companies require “an order” from your physician before they will pay for your medical visit. We recommend you call your insurance company to check on your benefits, and then you can call us to see if we accept your insurance.
